Contact your practitioner to complete and submit the required information to ESCO.

ESCO will send your practitioner authorization details.

Practitioner will send your device or impression to Manufacturer.

Manufacturer will ship device back to practitioner.

Practitioner will schedule time with you for fitting and adjustments.

* If your device is lost during the policy period, you will need to pay the balance of the premium before your claim is authorized.
